1. Purpose
Traction Consulting & Therapy is committed to protecting the privacy and confidentiality of the personal health information (“PHI”) and personal information (“PI”) we collect, use, disclose and retain in the course of providing occupational therapy and related services.
We comply with HIPA for PHI and PIPEDA (where applicable) for PI.
This policy describes how we handle information, and your rights in relation to your information.
2. Definitions
“Personal health information” or “PHI” means information about an individual’s physical or mental health, healthcare provision, or identification (e.g., name, health services number, address, date of birth) in the custody or control of a “trustee”.
“Personal information” or “PI” means any information about an identifiable individual that is not PHI.
“You”, “your” means the patient/client or individual whose information we collect.
3. Scope
This policy applies to all information collected by our clinic/practice in any format (paper, electronic, verbal) in the course of delivering our services.
4. Collection of Information
We collect only information necessary for the primary purpose of delivering occupational therapy services. This may include:
• Identification (name, date of birth, address, phone number)
• Health history, functional assessments, treatment plans, progress notes
• Billing and insurance information
• Contact information for family/support persons (when applicable)
• Information from or with other healthcare providers (with consent)
We collect this information either directly from you or from other healthcare providers with your consent.
5. Use and Disclosure
We use your information to assess your needs, develop and provide treatment plans, monitor progress, bill and manage services, and communicate with you and other relevant healthcare providers (with consent).
We will disclose information only as required or permitted by law (for example to regulatory bodies, as required by HIPA) or with your explicit consent.
We will not sell or rent your information to third parties for marketing purposes.
6. Consent
Your consent (express or implied) is required for most uses and disclosures of your PHI and PI. We may assume your consent in cases where you volunteer information for an obvious purpose.
You may withdraw your consent at any time in writing, subject to legal or contractual restrictions.
The primary purpose of collection is your benefit (treatment) — this is consistent with HIPA’s rules.
7. Access, Amendment & Requests
You have the right to access your PHI and request corrections/amendments to your information. If you believe your information is inaccurate or incomplete, please contact our Privacy Officer. We will respond as required under HIPA.
If you wish to request access, amendments, or make a complaint, you may also contact the Office of the Saskatchewan Information and Privacy Commissioner (OIPC).
8. Safeguards
We maintain appropriate physical, technical and administrative safeguards to protect your information from unauthorized access, disclosure, alteration or destruction. This includes:
Secure storage of electronic records (passwords, encryption where appropriate)
Secure storage of paper records (locked cabinets, restricted access)
Policies and procedures for staff and contractors, including confidentiality agreements
Regular review of security practices and staff training
9. Retention & Disposal
We will retain your information for the period required by law or professional guidelines, and thereafter securely destroy or anonymize it.
For PHI: retained for the period required under HIPA or professional/regulatory body requirements.
Records disposal will comply with best practices to prevent unauthorized access.
10. Transfers and ThirdParty Service Providers
If your information is transferred to a thirdparty service provider (external billing/administrative provider), we will use contractual safeguards to ensure that the service provider protects the information at least as strictly as we do.
If your information is being transferred outside Canada (or across provincial/territorial boundaries), we will inform you and ensure proper safeguards are in place.
11. Breach Protocol
In the event of a privacy breach (unauthorized access, disclosure, loss or theft of PHI/PI), we will:
Contain and assess the breach
Notify affected individuals and the OIPC as required under law
Mitigate any harm
Review and modify our policies and safeguards to prevent reoccurrence

This information will be used solely to contact you when appointments become available. If and when an appointment is booked, you will be invited to register through our Jane App platform, where your health and contact information will be securely collected and stored.
3. Use of Personal Information
Information collected through our website is used strictly for:
• Responding to general inquiries
• Managing our waitlist and notifying individuals when availability opens
• Transitioning clients to our Jane App system for formal intake and service provision
We do not use your information for marketing purposes, nor do we sell or share your personal information with third parties outside of service-related purposes.

All health-related information, appointment scheduling, and communication with clients occurs through our secure third-party platform, Jane App. Jane App is compliant with:
• HIPA (Saskatchewan)
• PIPEDA (Canada)
• PHIPA, HIPAA, or GDPR as applicable
Jane App provides secure encryption, user access controls, and data residency based on Canadian legal standards.
